adult information & registration
Anyone 18 or older who lives, goes to school, works, or takes part in programs in Avondale is welcome.
Step 1: Please read the information sheet linked below before registering for a workshop. The info sheet explains the purpose of the workshops and what you need to know as a participant. We do not need a signature for consent if you are 18 or older — participation shows you consent.
Step 2: Please register for a workshop so that we have a headcount.

youth parental permission & assent
Anyone 13 or older who lives, goes to school, works, or takes part in programs in Avondale is welcome. However, participants ages 13–17 will need permission from a parent or guardian to participate.
Step 1: The parental permission form is for one parent or guadian to fill out saying that a 13–17 year old can participate in the workshop.
Step 2: The youth assent form is for the 13–17 year old to fill out saying that they would like to participate.
Please print and fill out the form or email the form to d.j. trischler at trischdj@ucmail.uc.edu before attending. We will have paper copies at the event if you need them.
Step 3: Finally, register for a workshop so we have a headcount.
